Vaccine Information Statements

 

Vaccine Information Statements (VIS) are information sheets produced by the Centers for Disease Control and Prevention (CDC) that explain the benefits and risks of a vaccine to vaccine recipients.

 

Federal law requires that healthcare staff provide a VIS to patients, parents, or legal guardians before each dose of vaccine is administered.

 

It is very important that your facility provide patients with the most current and up-to-date VIS available for the vaccine(s) being administered. CDC provides access to the most recent versions of the VIS for each vaccine available. Healthcare professionals are also encouraged to sign up to receive email alerts from the CDC when new versions of VISs become available.
